Keyword Stuffing is the term to refer that the web content is stuffed with keywords or the keywords in the content in excess.
If a web content has keyword exceed to 5%-7% in a web content then it is consider as web spam due to keyword stuffing.

The keyword stuffing is depends on many SEO ranking factors. The repeated primary keyword on the prominence places such as meta tags, content, first paragraph and keyword density over 4-8% and many more ranking factors. It's completely Mal Onpage optimization process, it's can hurt the seo ranks.
 
Don't pay attention to the keyword percentage in your content, and write quality content. That's good SEO. You use your keywords when possible, but more importantly you have to make your content a GREAT resource for your visitors. You should be trying to increase returning visitors with good content.

I have seen some sites use duplicate keywords in the sites tag , but the site's ranking is very good, and very stable.

It is a properly formatted link strategy which will work if you set your domain as a clue, giving a title on your page, good contents on your page, what links are on your page and how will the search engines got there.
 
If a site content is stuffed with certain keywords then it lost its relevancy and also not liked by users. Keyword stuffing is a black hat technique and hate by search engines.
 
Keywords stuffing are when you write text that is filled with lots of keywords but basically no content in order to attempt to fool search engines into thinking that your website is about some subject. This is a really bad idea, though, because in writing a blob of keyword text, you have made your website less useful to humans, and how relevant your website is to humans is what any good search engine will rate you on. So, in short, make your website relevant and useful, and SEO will automatically follow.

I have seen many bloggers or webmasters trying to do keyword stuffing in the articles in order to get high rank instantly.
BUT they don't know that Google will soon penalize their site or will drop their ranking for sure.
It would be great If we could use keywords on such place where it needs to place. Otherwise Google is waiting to penalize you.
